Before you start completing the forms it would be helpful to collect the baptism and confirmation dates, church name and address for yourself and your family members. We ask these questions to ensure our records are accurate and complete. (Note: You can also save and continue your partially completed forms as well.) Due to the length of the forms they were split into three forms; one for yourself, one for your spouse or partner, and another for your children who are becoming members.
Pictures are part of our church and help to provide a historical record of events and participants at those events. Thank you for participating in preserving that history! Please note that the names of the participants are for historical records and will not be published without proper permission from the the individuals in the photograph.
Please complete the fields (Name, email, event and description are required). Up to 5 photos may be uploaded at one time. Each photo should be less than 10MB. THANK YOU!
